I attached screenshots showing the person responsible for this, including their username and evidence of their bad intentions. They had bragged about disabling accounts on their own Instagram story, and I included proof of this as well.

The following morning, I woke up and saw that my information had been verified. Instagram then prompted me to file an appeal, which involved submitting my ID. Make sure to use your full name as it appears on your ID, along with the email address linked to your Instagram account.
